Registration Policy
1. Payment in full must accompany registration form.
2. Registration is required by February 8, 2012. After this date, if application is accepted, late fee will apply.
3. No refunds can be obtained after the season has started.
4. L.M.B.A reserves the right to limit the number of players per team. Some exceptions may apply.
5. Applicants from outside the LMBA boundary will be placed on a waiting list. Upon evaluation of the teams, LMBA will advise the status of your application.
6. Player & Parent signature on team roster is required for all OBA teams. Details at www.baseballontario.com
7. N.S.F cheques will be assessed a fee, players will not be allowed to play until all outstanding monies have been paid.
Parental Consent to Play: I the undersigned parent or parental guardian of the above signed player, do hereby give consent that the minor listed above be allowed to participate in the Listowel Minor Baseball Associations (LMBA) sports program and agree to the registration policy. It is agreed that the LMBA executive and sponsors assume no responsibility for any injury that may occur as a result of this participation or travel to and from such participation. It is further agreed that all rules and regulations be abided by the participant and the parents.
